Loss of CLEO page on Form 4

Hi Group,

Hope someone can help me with this.

I was going through my original Form 4's the other day and noticed that the CLEO sign off (page 2) for my Reising was missing. I probably left it on a copier some time in the past.

Does anyone know if this can be replaced with request to BATFE? Note: Page one with "stamp" and approval is all there.

how many times has someone asked you for a copy of your form 4?

 

i have duplicates of all my forms...1 i keep in a safe, the other i keep in a binder that i carry with my guns

 

you should never have the opportunity to lose your form..(im done scolding you)

 

im sure the ATF can get you a copy....if you do a FOIA you will get a copy

Hi Bounty1,

 

With a written request, you can get a copy of your paperwork. IIRC, you need to send them your name, address, firearms description, S/N. You get a photocopy of your form returned (someday). 20 years ago you could even do this by FAX.

 

BTW, odd that they even approved the Form 4 on separate sheets. Normally they bounce. Otherwise I'd suggest looking at the back side of the page 1 :) At least you still have a stamp.

Group who replied regarding lost page of Form 4, i.e., the CLEO sign-off.

Believe me, I am very careful with the "original" Form 4's. They are kept under lock and key, and only copies with the firearms..

The forms were not submitted apart. This form goes back to the 1970's so there have been changes. The approval with stamp was on one page, and the CLEO sign-off on another. The original was returned to me as two copies.

How the 2nd page was lost remains a mystery, and I don't have any copies of it. If I did, there would be no problem.

To Grasshopper: Thanks for your input. The back of the first sheet just contains lots of BATFE boilerplate printed information. The CLEO sign off at the time, 70's, was on a separate sheet.

My worry was always keeping straight and/or losing the 5320.20 forms you have to file when you move. I'm military, so I've done a few of these in my time. And although ATF is USUALLY pretty good about getting these forms back to you in a reasonable amount of time, they have not always been. Couple that with having a C&R where you don't have to fill out 5320.20s for your C&R NFA items, and you can really get lost. There's probably not too many people that have that problem, but I'm guessing there are a few. It makes you get a little better about organization and record keeping, thats for sure.
